I thought it was a 3-button game, but here is a custom made Ikaruga cab, and it only has two buttons.

Aren't there supposed to be three (shoot, alternate colors, special shot)? The Gamecube version is three-buttons.

You are half right, it is actually a two button game that is commonly played with three buttons.
You can play it with only two buttons, and the instructions only show two buttons in the artwork and in the game demo. Button C is actually a shortcut for a quick A + B.
Most arcades will wire the 3rd button, but its not actually required to play the game properly.
(Also, the seller seems to have fuck all clue about the game as it won't run on any export bios)

you sure?? I am pretty sure that games works on any bios, but the text is in Japanese regardless.
